Licensed Social Worker II

Position Requirements:
1.)BSSW or equivalent required, 2) Current State of Ohio Social Work License (LSW or equivalent). 3.)Minimum of 5 years experience as a Medical Social Worker preferred 4.)Able to work in a diverse fast paced Acute Care setting assisting in the Care Coordination of patients across the continuum of care.
Role will assist the team in planning and coordinating care transitions, focus on high risk patients for readmit and complex medical discharges. Individual will manage their own daily case load. Individual will possess good interpersonal skills; have ability to work with multi disciplinary team. Must be team Oriented, Positive attitude -provide social counseling for patients and families in need.
This is a part time position and consists of 5 days, 8:
00-4:
30pm, per 2 week pay period. Weekend coverage on rotating basis with entire team and occasional off shift call is required.
Successful candidate will possess solid critical thinking skills, have ability to manage assignments independently but utilize the resources of the entire health care team in problem solving; work in collaboration with physicians and allied health providers, effectively communicate the discharge plan to the health care team in collaboration with patient and family goals.
